Funeral Arrangement

The person who makes the funeral arrangements must be able to give instructions and make decisions regarding the funeral details, this person will also be responsible for arranging payment for the funeral.

Information we need:

- Full name, address and age of the person who has died

- Location where the person who has died is resting

- Name and contact details of the doctor of the person who has died

- Whether a death certificate has been issued
